Returning Enrollment Documents Needed:

  • Current Immunization Record
  • Current Proof of Residency Documentation (Must verify your physical address on your child's enrollment packet w/your name)
    • Type of Residency Documentation:
      • NTUA Statement
      • Letter from your Chapter
      • Voter's Registration Card
      • Billing Statement
      • Driver's License (No PO Box)
  • Legal Documents, if any
    • Power of Attorney
    • Custody
    • etc.
